Recurrence risk stratification and long-term management of chronic rhinosinusitis: expert perspectives from the 2026 Zhongshan Forum

  • 摘要: 慢性鼻窦炎(CRS)是耳鼻咽喉科常见慢性炎症性疾病,具有病程迁延、临床异质性强、复发风险高和长期疾病负担重等特点。随着炎症内型、鼻内镜手术、生物制剂、数字化随访和人工智能(AI)技术的发展,CRS管理正从以症状和影像为主的经验化诊疗,转向以风险分层、精准干预和全生命周期管理为核心的综合模式。2026年慢性鼻窦炎中山论坛围绕难治性和复发性CRS、炎症内型转化、内镜鼻窦手术(ESS)与生物制剂治疗排序、基层规范化诊疗、智能化随访和科研转化等问题开展专题讨论。文章在整合论坛专家观点的基础上,结合国内外指南、立场文件和近年研究证据,提出CRS管理应强化复发风险前移评估、建立分层评估指标体系、优化传统药物、ESS和生物制剂的场景化排序,并以区域协同和数字化工具推动长期慢病管理。与会专家认为,CRS未来诊疗的关键不在于单一新技术替代既有治疗,而在于形成可执行、可评价、可推广的精准诊疗和连续管理体系。

     

    Abstract: Chronic rhinosinusitis (CRS) is a common chronic inflammatory disease in otorhinolaryngology and is characterized by a prolonged course, substantial heterogeneity, a high recurrence risk in selected patients, and a considerable long-term disease burden. With advances in inflammatory endotyping, endoscopic sinus surgery (ESS), biologics, digital follow-up, and artificial intelligence (AI), CRS care is shifting from an experience-based model relying mainly on symptoms and imaging toward integrated management based on risk stratification, precision intervention, and life-course disease control. The 2026 Zhongshan Forum on Chronic Rhinosinusitis discussed difficult-to-treat and recurrent CRS, clinical translation of inflammatory endotypes, sequencing of ESS and biologics, standardized primary care pathways, intelligent follow-up, and translational research priorities. Based on expert discussions from the forum and current guidelines, position papers, and recent evidence, this article proposes that CRS management should strengthen early recurrence-risk assessment, establish a tiered evaluation system, optimize scenario-based sequencing of conventional pharmacotherapy, ESS, and biologics, and promote long-term chronic disease management through regional collaboration and digital tools. The experts emphasized that the future of CRS care lies not in replacing established therapies with a single new technology, but in building an executable, measurable, and scalable system for precision diagnosis and continuous management.
